If you are currently in hospital, reablement care can be organised for you through the NHS by the hospital discharge team, but if you would like more flexibility in your recuperation, our Care Professionals can provide compassionate, adaptable reablement care in the comfort of your home so you can hit the ground running with your recovery.

Unlike traditional home care, which provides ongoing assistance, reablement care is time-limited and goal-oriented. It supports recovery and focuses on helping individuals relearn skills or adapt to changes following a setback, rather than carrying out tasks for them. The ultimate aim is to empower individuals to manage independently as much as possible.
Reablement care is often arranged through your local council after a hospital discharge or health assessment. Speak to your GP or hospital team to request an evaluation, or contact your local adult social care service. If eligible, the council may provide a reablement package free for up to six weeks, depending on individual circumstances. Alternatively, give your local Home Instead office a call for a chat about reablement home care services fully tailored to your needs.
At the end of the reablement care period, the individual’s progress is reviewed. If further support is needed, options such as ongoing home care, occupational therapy, or community support services may be recommended. Some individuals successfully regain independence and no longer require additional care, but we’ll work closely with you to make sure you always have whatever support you need.
