Their rapport with him not only speaks of quality care but of genuine connection. Continuing to be supported by the original office team, including his same Home Instead Field Care Supervisor, has maintained a continuity of care. His multidisciplinary healthcare team, including occupational therapists and other healthcare professionals, work alongside us in a coordinated effort that exemplifies collaborative care at its best.
