The result? Instead of an unfamiliar care home setting, our client returned home, and is now supported by two Care Professionals operating on a carefully considered rota system. Their rapport with him not only speaks of quality care but of genuine connection. Continuing to be supported by the original office team, including his same Home Instead Field Care Supervisor, has maintained a continuity of care. His multidisciplinary healthcare team, including occupational therapists and other healthcare professionals, work alongside us in a coordinated effort that exemplifies collaborative care at its best.
We understand that the warmth of home, the proximity of family, and personalised care markedly influences recovery.
