An unexpected stay in hospital can be distressing and it can really knock our confidence when returning back home. Home Instead Wrexham & Flintshire has a great deal of experience in helping people to return home from hospital safely, whilst assisting to rebuild their confidence and sense of independence.
We offer a range of support services to assist with your recovery, from organising transport home to arranging equipment to aid your mobility and independence. Our care professionals can offer help around the home, as well as making fresh, nutritious meals leaving you to focus on getting better.

Whether you are being discharged from Wrexham Maelor or one of our community hospitals in Mold, Chirk or Deeside, we have experience of working with the community health teams, district nurses, occupational therapists and a range of other health professionals.
Working together with you, your family and your healthcare team we can provide you with a bespoke package of care to aid your recovery.

We offer two types of home care: hourly care, where we visit at set times, or live-in care, where a carer resides in the home. Both are overseen by our care management team and delivered by compassionate Care Professionals. Each care package is made up of of a unique mix services to meet your needs.

Do I need to wait for the hospital to arrange a care package?
You don’t need to wait for capacity to become available from your local social services provider, care from Home Instead Wrexham & Flintshire can be arranged directly by yourself, your family or you may ask the hospital discharge team to contact us on your behalf.
If appropriate, we can come and offer our care consultation in the hospital to assess your initial needs – with further reviews once you are home to ensure that your care remains relevant to your changing needs. You may find that following your recovery you wish to decrease your level or care, or you may want to keep your regular visits – we will ensure that you have the care you need, when you need it.
