Long-Term Injury Case Management in Enfield

When someone is living with a long-term injury or a complex health condition, day-to-day life may require additional support. Alongside clinical input, many people also need consistent help at home, support with routines, and reassurance that their care is joined up.
Home Instead Enfield works alongside clinical case managers, healthcare teams, and families to provide relationship-led home care that fits around each person’s needs, goals, and preferences. Whether support is needed after hospital discharge, during rehabilitation, or to help maintain stability at home, we can build a flexible package that feels practical and well-organised.

Who This Support is For
Clinical case management companies often coordinate care for people with complex needs, where the right support at home plays an important part in progress and wellbeing. Home care may be helpful for clients living with:
- Traumatic Brain Injury (TBI) or Acquired Brain Injury (ABI)
- Spinal cord injury
- Amputation
- Multiple Sclerosis (MS)
- Motor Neurone Disease (MND)
- Huntington’s Disease
- Cerebral palsy
- Dementia with additional health needs
- Long COVID with complex needs
- Mental health needs alongside physical support requirements
Every situation is different, so we focus on understanding what matters to the individual, what outcomes are being worked towards, and how care at home can support those goals day to day.

How Home Instead Can Complement a Case Management Pathway
Case managers are often coordinating multiple services, appointments, and professionals. Our role is to provide reliable home care that supports the wider plan, supports families with day-to-day care needs, and helps maintain a steady routine at home.
This can include:
- Consistent personal care and daily living support, delivered with dignity and respect
- Medication support where appropriate
- Meal preparation and hydration prompts
- Mobility support and safe assistance with transfers, guided by care plans
- Practical help around the home to keep routines manageable
- Friendly companionship and reassurance, which can make a real difference to confidence and motivation
We aim to bring calm, steady continuity to the home setting, so the person feels supported, known, and comfortable with the Care Professionals around them.

Delegated Healthcare Tasks and Clinical Support in The Home
Some Home Instead offices are able to undertake delegated healthcare tasks where training, governance, insurance, and oversight are in place, and where the commissioning professional permits. These can include routine personal care activities such as PEG care, catheter care, and stoma support.
In England, Wales, and Scotland, Care Professionals may also support with more complex delegated clinical tasks where appropriate planning and oversight are in place. Examples may include:
- Nebuliser support
- Wound care support (including dressing changes and basic pressure care)
- Clinical observations (such as blood pressure, blood glucose, temperature, pulse, respiration)
- Oxygen therapy (cylinder or concentrator)
- Non-invasive ventilation (NIV), with clear care plans and oversight
- Buccal midazolam
- Prescribed bowel care interventions where appropriate
- Tracheostomy care
- Topical negative pressure therapies (if supported)
This list is not exhaustive. Any clinical tasks must be agreed, delegated, and supported through appropriate clinical governance and competency processes.
Important note for Northern Ireland: the Regulation and Quality Improvement Authority (RQIA) does not currently permit Healthcare at Home services to be delivered by Care Professionals. Offices in Northern Ireland must not undertake delegated clinical tasks unless they have express permission from their regulator.

Flexible Packages: Hourly Care and Live-in Care
Complex packages often change over time, especially during rehabilitation or following discharge. We can support with:
- Hourly care, from short visits through to more regular daily support
- Live-in Care, offering continuity and reassurance for clients who need a consistent presence at home
- A flexible approach that can step up or step down as needs change
For some clients, Live-in Care can help create a steady routine and build confidence before transitioning to a different level of support. For others, a carefully planned hourly package may be the right fit from the start.

Working Collaboratively with Case Managers and Wider Teams
Joined-up communication matters. We are used to working alongside professionals involved in a person’s care and can support practical integration with the wider plan, including:
- Following structured care plans and risk assessments
- Supporting goal-based routines at home, aligned with therapy recommendations
- Attending agreed MDT discussions or meetings where appropriate
- Keeping clear records that support continuity and oversight
- Being open to client-specific training and agreed competency frameworks
Our aim is to make care delivery dependable and transparent, so case managers feel confident in the home element of the package.

“We haven’t supported this exact client profile before”
It’s completely understandable to want reassurance when needs are complex. Home Instead is a national network, and even if a local office is building experience with a particular condition, the wider organisation shares learning, training approaches, and case study examples that help strengthen planning and confidence.
What matters most is having the right governance, a willingness to work to the clinical plan, and the commitment to train and support Care Professionals properly for each individual client.

Helpful Questions For Early Conversations
When exploring a potential referral pathway, it can help to clarify a few practical points early on:
- What type of home-based support is needed right now, and what outcomes are being worked towards?
- Are there any delegated clinical tasks required, and what training and oversight process is preferred?
- Is continuity a key concern for this client, and what has worked well for them in the past?
- Would Live-in Care be useful at this stage, or is an hourly package more appropriate?
- How will progress and day-to-day observations be shared with the wider team?
These conversations help ensure care is set up in a way that feels organised, realistic, and supportive for everyone involved.
