Palliative and End-of-Life Care at Home in Monmouth, Abergavenny & Pontypool

Compassionate palliative and end-of-life care at home across Monmouth, Abergavenny, Pontypool and rural Monmouthshire, from a local, trusted team.

When someone you love is living with a life-limiting illness, the questions come thick and fast. How much time is there? What will they need? And, very often, the most important one of all: can they stay at home? For most people across Monmouthshire, home means everything – the kitchen where the family has gathered for decades, the garden they’ve tended, the view of the hills from the bedroom window.

At Home Instead Monmouth, Abergavenny and Pontypool, we help make that wish possible. Led locally by Karen and Laura Clatworthy, rated 9.6/10 on homecare.co.uk and named one of the Top 20 home care providers in Wales, our team provides gentle, experienced palliative and end-of-life care in homes across Monmouth, Abergavenny, Pontypool and the villages in between.

What palliative and end-of-life care mean

The two phrases are often used together, but they describe slightly different things:

  • Palliative care – support for someone living with a serious illness that can’t be cured. The focus is on comfort, managing symptoms and quality of life, and it can last for months or even years.
  • End-of-life care – the care given in the final weeks, days and hours of life, when comfort, dignity and being present matter most.

We support families through both, and care can grow gently as needs change – from a few visits a week to overnight care or full live-in support.

Why so many people choose to be cared for at home

  • Familiar surroundings, routines and belongings bring comfort and calm
  • Family, friends and pets can be close at any time of day or night
  • Favourite foods, music and small everyday pleasures stay part of life
  • There’s privacy and peace that a busy ward can’t always offer
  • Your loved one keeps a sense of control over how their days are spent

What our palliative care includes

Every care plan is shaped around your loved one’s wishes and your family’s needs. Depending on the situation, our support can include:

  • Personal care with dignity – gentle help with washing, mouth care, dressing and continence, at a pace that feels right.
  • Comfort and positioning – helping your loved one stay comfortable in bed or their chair, and changing position regularly.
  • Food and drink – small, tempting meals and sips of favourite drinks as appetite changes.
  • Medication support – reminders and support in line with the care plan, and making sure the right people are called quickly if symptoms change.
  • Overnight and live-in care – sleep-in, waking night or round-the-clock support, so no one has to face the night alone.
  • Companionship – reading aloud, playing favourite music, talking about old times, or simply sitting quietly together.

Working alongside your loved one’s healthcare team

Our Care Professionals don’t replace nurses or doctors – we work alongside them. That usually means your loved one’s GP, the district nursing team, palliative care nurse specialists, local hospice teams, and charities such as Marie Curie or Macmillan where they’re involved.

Many people nearing the end of life have “just in case” medicines kept at home. These are given by the nursing team, and our Care Professionals make sure the right contact numbers are always to hand and that help is called promptly if symptoms change.

Palliative care in rural Monmouthshire

Living in a village or on a farm shouldn’t mean going without support at the end of life. Our Care Professionals live across the county and are used to reaching homes down country lanes and in the hills. For families in more isolated spots, live-in care can be a particularly good option, giving continuous support without anyone having to travel back and forth.

Wherever possible, the same small team visits every time. At a time like this, a familiar face at the door makes a real difference – to your loved one and to you.

Support for the whole family

Caring for someone at the end of their life is one of the most demanding things a family will ever do. Our support is for you too:

  • Time to sleep, eat, rest or step outside for some fresh air
  • Help with meals, laundry and keeping the house running
  • Someone calm and experienced to talk to at difficult moments
  • Signposting to local bereavement support when you need it

Planning ahead makes a difference

If you can, it helps to talk about wishes early – where your loved one would prefer to be cared for, and what matters most to them. Their GP can help put an advance care plan in place. Setting up home care before a crisis also means your loved one can get to know their Care Professionals while they’re still feeling relatively well.
