Medication Support at Home in Monmouthshire

Safe, reliable medication support at home across Monmouthshire – reminders, help with doses, repeat prescriptions, and every dose recorded.

A pill organiser on the windowsill. A carrier bag of boxes from the pharmacy, some of them finished with months ago. A list of instructions that changed after the last hospital stay and never quite got written down properly. For a lot of older people, medication becomes one of the hardest parts of daily life and for families, one of the biggest sources of quiet worry.

At Home Instead Monmouth, Abergavenny and Pontypool, medication support is part of what we do every day. Led locally by Karen and Laura Clatworthy, rated 9.6/10 on homecare.co.uk and named one of the Top 20 home care providers in Wales, our trained Care Professionals help people across Monmouthshire take the right medicines at the right times, safely.

Why it gets harder

  • More medicines at once several conditions means several prescriptions, each with its own timing and rules about food.
  • Changes after hospital doses get altered, new tablets are added, old ones stopped, and the leftover boxes stay in the cupboard causing confusion.
  • Memory changes it becomes easy to forget a dose, or to take one twice because you cannot remember the first.
  • Eyesight and dexterity small print and childproof caps are a genuine obstacle when arthritis makes packets hard to open.
  • Time-critical medicines for Parkinson’s care in particular, doses need to land at precise times. Half an hour late can mean a noticeably worse afternoon.

Signs a loved one may need help

  • Tablets left in the organiser at the end of the day, or boxes running out far too quickly
  • Uncertainty about whether today’s dose has been taken
  • New drowsiness, dizziness, confusion or falls
  • Repeat prescriptions not ordered, or collected late
  • Tablets found on the floor or down the side of a chair
  • Symptoms that had been well controlled starting to return

How our support works

The level of help is agreed in the care plan and reviewed regularly. Depending on what is needed, it can include:

  • Reminders a prompt at the right time for someone who manages their own medicines but occasionally forgets.
  • Practical help opening packs, pouring liquid medicines, applying creams and eye drops.
  • Giving medication where this is agreed in the care plan, by Care Professionals trained to do it.
  • Recording every dose so there is always a clear written record of what was taken and when.
  • Keeping on top of supplies checking stock before it runs out, and helping order and collect repeat prescriptions.
  • Raising concerns telling the family, GP or pharmacist about side effects, refused doses or anything that does not look right.

The rural pharmacy problem

In a town this is straightforward. Living in a village it is anything but. If the nearest pharmacy is a drive away and your loved one no longer drives, a repeat prescription becomes a logistical exercise and one that quietly gets skipped.

Our Care Professionals can collect prescriptions as part of a visit, which for many of the families we support across rural Monmouthshire is one of the most useful things we do. It is also worth asking the pharmacy about delivery, as many will deliver locally.

Safe, trained, properly overseen

Every Care Professional who supports with medication is trained to do so, and our Registered Care Manager, Nesta Bishop, oversees every care plan. Home care in Wales is regulated by Care Inspectorate Wales, and medication is one of the areas we take most seriously. If a dose has been missed or something is unclear, our Care Professionals do not guess they follow our procedures and seek advice from the pharmacist, GP or NHS 111.

After a hospital stay

Coming home from Nevill Hall, the Grange University Hospital or elsewhere almost always means a changed medication list. Our Care Professionals can work through the discharge letter, set aside anything that has been stopped so it does not get taken by mistake, and make sure the new routine starts properly from day one rather than drifting for a fortnight. This is particularly important for someone recovering from a stroke, where medicines to prevent another one need taking consistently.

Worth asking the pharmacist or GP

  • Can the medicines be supplied in a dosette box or blister pack sorted by day and time?
  • Can the pharmacy deliver to the village?
  • Is a medication review due? Where someone takes a lot of different medicines, a review can often simplify things.
  • Can repeat prescriptions be set up electronically to reduce the chasing?

We are always happy to help families raise these questions, and to be there for the conversation if that helps.
